back wheel moving around

almost there on my build. not liking the levers but I can change those. what I'm struggling with is I cannot stop the back wheel moving around. the wheel nuts look like that are damaging the frame and even so I still cannot stop the inner cone moving and the wheel sliding forward. any ideas?
