I only ever used Cooks Cranks bitd and never had end caps on them.

Those Cook Bros Racing cranks pictured are the later ones usually made for the MTB market and yes, mountain bikers might have run crank caps on them to keep the mud out. Early Cook Brothers Racing (italicy font) BMX cranks didn't come with a cap, the axle bolt was quite big and yes the threads were mainly for a crank extractor.
