Cheers all.
The trick is a no handed swivel.
You do a normal swivel but rest the peg on the floor and stand up.
Make sure you keep your weight back or you'll go over forwards. A freecoaster helps but isn't essential.
Have a go and see how you get on!

Some more digging and I found there are angled cable adjusters/tubes and elbows for motorcycle carb throttles.

You might find one that fits.
