Do these things just push into the steerer tube and sit above the welded in compression nut?   Or do you use one if there is no welded compression nut?

Only if there is no welded nut glen.........and they need a good whack to get them in.

Maybe the thread is fooked on the forks so they wacked one in?

You might be able to push it all the way through, Or get a bolt in it and a claw hammer, Use something to protect the top of the forks.

pickle:
I normally use a big screwdriver and hit the flanges with a hammer.  breaks the individual flanges and it loosens up so it can come out.
